Before Your
Appointment

The initial tests ordered by our providers are customized according to each patient’s recent lab work and the patient history/symptoms gone over with the new patient coordinator, Ariel. Please read thoroughly and if you have any
questions, you may email Ariel. If you have not had a phone call with Ariel, please email info@crozierclinic.com or call 407-732-7668 to schedule your call.

Prior to your appointment, we ask that you send us:

• Any patient history you may have
• What symptoms you would like to address with Dr. Crozier
• Any past labs that may indicate what you are suffering from

In preparation for your appointment, please complete the following:

• Bring any supplements or medications you are currently taking for review to the appointment
• Complete your new patient paperwork and send back to us before your consultation. If you are not able to do
  this, please arrive 10 minutes early to give to the front desk.
• Have any important questions written prior to your appointment to ensure they are answered and provide
  clarity for your individual care.
• Write down any additional information you feel is necessary to your case.
